Who shall be liable for Sign form 16A & B on traces

One of my client came to me belong from teaching profession govt job.
His form 16 a signed by a consultant and in 16b the salary showing by him only 1 rupees.

Could you pls suggest me how can I file the ITR of such person?
Is this valid that a consultant can sign 16a and B on behalf of DDO?
Very personal last one how much fees do I have to charge for this...If I can file TDS return?

TDS return or ITR?

If it is not happen than any panality in this case.

Difficult to prove any irregularity, leave aside any penalty.
