which forms to fill after completing articleship..........

My articleship is completed this 22th jan. I want to know which forms to fill and where to send them?? I also want to know that till which date i can do this?? any cut off dates for it?? pls help me..............i m doing self study and no coaching, so pls help me regarding what next after completing articleship...............
