Wheathe certification required in dealer invoice

Dear Friends,

We are puchasing some good from 1st stage dealer and in excise invoice there is a portion with heading "certificate"((a) Imported directly by us or received directly from m/s............... (b) recierved directly from the factory / depot / consignment agent / authorised premises of M/s...............................(c) Received from M/s................).

which is left blank by him, will this lead to disallowance of cenvat by the department.
