What is e-grievance form application - enivaran - incometax

Dear Sir/Madam,

I was exploring eFiling options under Grievance e-Nivaran section.

Under files to be uploaded it says eNivaran form application. Is there any form for the same, or are they expecting an application written and signed by the user on plain paper for the context applicable? See the attached screenshot.
