I have a small query regarding Surrender of Service Tax Certificate of an Assessee, whose original ST 2 has been misplaced, is there any alternate solution to surrender the ST-2

Inform dept for the same. No penalty wud be levied for the same. A declaration may suffice.
I had misplaced the original ST-2 Certificate and want the surrender the service tax number. please any one send me the formalities to be completed in this regard.

File a FIR to your near police station that your ST-2 has been lost and submit the copy of FIR with surrender of service tax application to the service tax department.
