just write a hand written apllication with prescribed fees of the papers u wan tto do revaluation there is no subject code & no standard format format it just write simple application & write on it the paper of which u want to get it revalue
this file will solve your all querie's
but dont forget that this is performa only and you have to sent a handwritten application to icai with a 100/- rupee draft for each subject and 400 maximum
