Vat/ cst registration

Hello All,

               I am a new Priprietorship firm in Mumbai and I am trying to apply for VAT/CST, I do not have turnover of Rs 5 lakhs yet so I need to apply Voluntarily. Now when i am trying to apply for Vat/Cst online it is asking for Voluntarily Registration, Introduction to a registered dealer (should be continually registered for 5 years) rule 8(11)(C).

Asking:

1) Name of the Dealer

2)Tin under MVAT Act 2002

3)Date of effect.

Can anyone assist me with this? Or I need to spend money and get one from a agent and ask him to complete the vat registration for me.
