What is Local H Form & OMS H Form??????

As per my study i come to solution that

 

when local dealer give H form, then it is ' local H form'.

 

&

 

when out of state dealer give H form, then it is 'OMS H form'.

 

This information is required in 704 form MVAT audit.

 

if i m wrong plz tell me.
