Those reports are very sensitive. and it is very hard to be online or from public source.

i have tried very much ealrier. but could not get any significanty one.

What Amit is saying is true. But I can give you brief guidelines to prepare it.

Contents would be: Cover page, Covering letter, Index, Executive summary, Observations and Conclusions.

Report should stress more on controls and weaknesses rather than accuracy of accounts like statutory audit report. Recommendations always add credit to the report.
