Unable to file Spice+ Web Form

I am not able to download Spice Form for signing after submitting same, what could be the possible reasons and how can it be resolved. Kindly guide me in this.

Spice+ part A was filed for name approval and even the name is approved but unable to proceed further for Spice+ part B. When we click on fill option of Spice+ for proceeding further for part B ONLY Auto-check option is showing after name approval PROCEED option is not showing attaching name approval letter and screenshots from MCA portal.

 

If anyone knows kindly help
