What is the procedure for transferring funds (savings or fixed deposit) from NRO bank fixed deposit account of an NRI to his NRE fixed deposit account ?

Fill up the firm and transfer. No need to contact anyone.
There's no need for anything special if the transfer is not related to a business or professional income earning a activity.
If at all, the bank will ask for an explanation. And if the bank doesn't need anything then you don't need to worry either.
