To fill and chose the itr form

when i was at 19 (2011) i stated business in HP having tin no for my retail business dont know about any thing how to choose form how to fill it all work done by my accountant  he file my ITR which is Form 5 after 4 yeras i sale my business and shifted to my home town Muzaffarpur Bihar and and complete my MBA  (2015-2017) right now my earing is from ambulance which i purchased in dec 2014 from till date i have earning from my vehicle so which form i go to file it ..... kindly suggest
