TDS return error for unique details.

The correction statement for 24Q/4Q got rejected post filing (offline). The reason cited is 'Deductee Details not unique for addition of new deductees'. It clearly has something to do with newly added deductees in the correction statement but what is mean by Deductee Details? What details should I look for which needs to be changed/corrected (which needs to be unique) in the correction statement to get rid of this error?
I cannot even upload fresh request for conso file now as this correction statement has got rejected.
Please note, I used software for preparing regular and correction statements and not NSDL RPU utility.
