Surname change in pan card

Hi,

I am trying to change the surname in my mom's PAN card. I am stuck with arranging the proof for the same.

My mom's PAN card has her father's name as the surname but in all other accounts(savings, pension, MF, ration card etc.) it is our family name(got changed after her marriage). Since the details in her PAN card is different it is leading to lots of troubles these days(like name mismatch in MF, unable to open demat account).

I submitted a modification form to update details of her existing PAN card. I also annexed her savings bank account xerox, ration card xerox as proof for the same. But the agent says it wont pass through because all the proofs does not have her surname(our family name) expanded. Now the agent is asking for me to get the name changed through a gazzeted officer.

Is this the only way or is there someother procedure for the same?
