Hi Friend
you can download Form ST1 from Service tax site.
Then Fill Name add, phone no, nature of Services etc. the filling of form is quiet easy
1. Service Tax Registration:
no fee has to pay.

I Owned on Sole Proprietorship Concern, I will get service tax reg in my name or concern name

Registration will be taken in the name of proprietorship concern.
