Service Tax - Insurance Sector

I have a query on the relevant address for service tax purposes in an Insurance sector if the addresses for the following are different and what if any of the address is in the state of Jammu & Kashmir.

Policyholder has the following address:

Policy owner address (Permanent)
Policy owner address (Communication/dispatch)
Policy insured address:
Payer address:
Beneficiary address

Kindly note the following

Policy Owner is different from policy Holder.

Policy Holder is the one who has been insured

Payer may or may not be policy holder/owner

Beneficiary is the one who would be benefitted from the Policy
