Dear Anil,
1) You will have to register online and then submit the signed copy of ST-1 with the necessary proofs.
2) I think so the Service Tax Department does not accept manual filing of ST-1.
3) You will have to be registered under the 'Business Auxillary Service' Category. Service Tax will be levied once your Gross Receipts exceed the specified limit (ie. Rs. 10 Lacs)
