Self declaration to be submitted during exam

I have self declaration form but attachment option is not available here

As per Admit Card , No self declaration is required in case major aged students, Self Declaration is required only in case of minor aged students.
But If still we need to submit any self declaration then please give the format in which self declaration need to be submit, Because on this Sunday I also have my first exam of second group. 

I confirmed from one of my friends who gave 1st group exam, no self declaration form required.

Only in the case of opt out , self decoration form is required
