file amendment to your old pan card, by filling 49A, and send the affidavit copy/ paper publication xerox, bank account name change (pass book copy) and other documents to NSDL , they wil send a new pan card in new name but with old number.

There is a panelty of rs.10000/ if a person retains more than one pan card . you can make correction in your exexting pan card by filing correction pan data form.

Keeping Two Card is Subject to Penalty.............You Can Fill Form For Changes Or Correction in PAN Data Physically by Downloading Or Filing Online.

Surrender old one then apply for new one

Do it as soon as possible, because if you file your returns and other tax works with new Pan no. and in future you will go for the same to NSDL/UTI for cancel old one.
There may be chances they harrash you by the reason: Meet the ITO with whom you have your file and satisfy him that you have not taken any advantage or misuse your new pan no in place of old one.
as the time passes in future you will face difficulties for changing also.
and for avoiding penalty you are to fill form 49A for changing in Pan name.
