Rg 23a, 23c and service tax

Dear Friends,

I need to know what are the details which the accountant should fill in all the three annexure sheet.

I have a doubt in Service tax annexure.   Whether input and output both will come?  or  Input will come?....

Further, Tariff no. for all the Three categories.   My client is not storing these data in these ERP.  

Hence, finding difficulty...
