Who will issue Form 10E ?.

You have to file the Form 10E at the time of filing Return. That means, you need to file 2 return forms to department if you claim relief U/s 89(1).
Nobody issues Form 10E.
if you received arrears of salary then you need to file 10E online through log in on Portal before the due date of filing ITR.
You can claim relief under 89 (1) only if you file 10E in time.
