Due to clerical error wrong account no. has been given in Income tax return for refund.
And such default is come into knowledge after refund has been encashed into wrong account.

Update account no. and now the person in whose account its been encashed ask him to give you back
Actually problem come into knowledge after 7 months of refund encashment.
