is it necessary to take approval of rbi before change of registered office from one state to another state.
if yes, kindly provide me any format of the same.

It is not prescribed under any provisions of CA, 2013 and rules made there under, requiring company to take permission/approval from RBI for shifting of registered officee from one state to the other, hence, your company is not required to take any permission/approval from RBI.
