Query regarding certified copies of answer sheets

Dear All

While applying for online for "supply of certified copies/inspection of evaluated answer books" facing following queries-

1) Whether we are required to submit?

a) scanned copy of our signed handwritten request.

b) scanned copy of the duly signed Declaration.

C) Or both?

Please let me know on urgent basis....i shall be highly obliged to you.
