Query for rectification of examination form

Dear sir/Madam, I am a graduate converted to IPCE from CPT on 23/10/2012 and i already completed my articleship on 23/03/2016. at the time of filling examination form instead of selecting direct entry scheme -Graduates - converted from CPT (Point no 10) i wrongly selected Point no 13 (iii) (b) -direct entry scheme intermediate level pass of ICWA/ICSI-converted to intermediate from CPT. hence it didn't ask for certificate of Service. i proceed with the payment. later i came to know the mistake . please suggest me how to rectify the examination form.
