Query for filling form 10e for 2 subsequent years

Hi i have a query because the 10E form i filed in this AY 2018-19 has been rejected by IT dept. My query is i did not understand why it has been rejected.

I have filled 10E in AY 2017-18 and AY 2018-19 both and last year it was accepted but this year it is rejected.

Now my query is

1. Do i have to enter details of FY 2016-17 and FY 2017-18 both in the 10E form of AY 2018-19 or only the details of FY 2017-18? (I filled only details of FY 2017-18)

Please help me out in understanding the reason of rejection. Thanks.
