Firstly you should contact  your Regional Office in which you are registered with your old Registaration No. They give you your New Regisatration no(i.e ERO,CRO like this.) Transition scheme is there for conversion form old couse like (Intermediate to PE II, PE II to PCC and PCC to IPCC). So Please contact your regional office they can guide you better.
