Proof under Section 80DDB

I am claiming tax exemption under Section 80DDB for my father who has Dementia. The doctor has mentioned that he is ready to sign the certificate but he is not ready to classify the disability percentage as there should be specific institutions to classify the disability percentage.

Can anyone please let me know which form I need to add while filing my returns and who can sign that in Bangalore?

I think it should be Form 10I but am not sure.

To my knowledge all that forms are not required while filing itr but that proofs should be with you
You can file ITR, no attachment is required but proofs should be with you

You can claim deduction
In your case you will take the certificate from neurologist having a doctorate of medicine degree in neurology or any equivalent
degree which is recognised by the medical council of India as your father is suffering from neurology disease
again if you're father treatment is done or going on in private the only private doctor gives certificate no government doctors
the certificate must have name and age of the patient
name of the disease or ailment
name address registration number and the qualifications of the doctor issuing prescripttion
and if in government hospitals then the name and address of the hospital
if you want you can file form no 10 -1and verified by specialist or doctors with medical council recognized degree
either above certificate or form no 10- 1 are to be submitted to the income tax department for claiming the Deduction under 80DDB

Thank you for the detailed suggestion. Form 10i talks about percentage disability in case of neurological diseases. The specialist doctor is saying that there are agencies to do that assesment and he may not be the right person. Hence am checking. Should I check witha different doctor?

I got the form signed by the specialist. Do I need to show actual bills now?

I'm sorry was not clear. I meant apart from the form 10IA filled up by the doctor that my dependant father has dementia with >40%disabilty, do I have to upload actual bills of medicine etc. to claim the deduction of 1 lakh? 

My organization is asking for the bills to be uploaded baling with the certificate from the doctor.

as per my knowledge no medical bills is attached with the certificate 

Medical bills and prescriptttion are kept for future reference if needed by AO officer while submitting the form to claim Deduction  otherwise certificate of a specialist doctor is enough to give a organisation.
