Plz..Help me Urgent

I am a PCC student & has completed my articleship period & submitted "FORM 108" to the Institute but I could not clear my Group-II. So, I will appear for the same exam this Nov.,2010. Do I need to submit Declaration form named as "DECLARATION BY CANDIDATE" attached alongwith FORM B of exam form. As the same needs the Signature of the Principal for the candidates who are presently under going the articleship, but I have already completed the articleship period.

 

OR

 

I can submit the exam form as 1)FORM A- OMR Application Form

                                             2)FORM B.

 

Please suggest me a right solution.
