Is it true that the institutes fixes pass % for each and every exams

Students will tell you that they do. The Institute will say that they don't.
I personally don't think they "fix" a specific pass percentage. It is more likely that in certain years they do try to tweak the pass rate higher or lower, as they feel necessary.
