Pan for sole proprietorship

Hi:

I already have a PAN number and wish to apply for a new PAN number of my Sole Proprietorship Firm (in the name of the Firm). While filling form 49A online at tin.nsdl.com, under the "Category of Applicant" which is asked before filling the form, I selected "Body of Individuals" and proceeded to fill the form. The acknowledgement was generated, I attached my residence proof and PAN number photo-copies as proofs and couriered the documents. Following is the email reply received from the ndsl side after verification of documents:

Dear Sir / Madam,

We refer to your abovementioned application & PAN application documents received from you on August 12, 2015. On verification of your application and PAN application documents, the following discrepancy(ies) is/are observed by us:

Proof of address is not provided with application form.

• Submit proof of address where name in application exactly matches with name in proof.

Proof of identity is not provided with the application.

• Submit proof of identity where name in application exactly matches with name in proof.

Now the proofs of residence and identity are in my name only and I have no document in the name of the sole proprietorship firm name. Kindly suggest what needs to be done.
