I am applying for Re-print of my lost pan card. I remember the site accepting Bank Passbook as Proof of address and now for re-print I can't find the option in the list. Instead the option is changed from "Bank Account Passbook/Statement" to "Bank Account Statement". Online downloadable statement of my bank account is it valid for this option of Proof of address ? or should I get it from the bank directly ?

Dear sir
If online statement is showing proper details of your address then you can present it. (should be self attested).
I also suggest you to give your voter id copy as proof of address.

Yeah the Statement shows my address. I have attached a sample statement. By saying self attested you mean i have to put my signature on it right ?
Right now My voter ID is not having the details correct. I have submitted it for correction.

Online bank statement will be accepted only if there is seal of bank on it. So avail bank stamp on copy of bank statement
