no worries, just do any of the following 2 steps
1. Go to institute and correct the same
2. Don't go ICAI, On examination day carry your valid photo id proof

This is a most common thing that happens while issuing the Admit Cards.
What the student need to do is take a print out of the Admit Card and then attach a photo in the blank space and sign it. Now, this is to be attested by a practicing Chartered Accountant.
It is also recommended to carry an ID proof to the examination center.
Proof mean, any one that is issued by an govt agency. Eg: PAN card, Aadhar card etc, ICAI Articleship I'd card etc
