Need jobwork delivery challan format

Dear all,

We are engaged in the Leather Goods manufacturer industry. We are Sending Materials to the Jobworkers ( Like Leather , Lining, Fittings etc ) To conevert in to a Finished Product as Well as Semi finished.. For This we require the Challan Format for the Belwo :-

1. Jobwork challan format for sending goods to the Jobbers

2. Challan Format when the Jobbers returned the Goods after Converting in to Finished product.

3. Format of Invoice for Service Charges to be billed by the Jobbers.

Also What is the Procedure for Interstate & within state  jobwork. And also confirm that the HSN code is must in all the above three stage..
