need help for filling ipcc exam form - converted from pe2

dear frends, i m pe2 student. i converted my self to ipcc on 06/08/10. In exam form the point given for deciding eligibilty status contain the options with condition that whether converted on or before 01/02/09.

so someone who have converted after result please help me whether to fill that option or not. also what abt cct details , are we required to attach cct certificate copy.
