I have allotted more than one PAN. Hence I am going for PAN Correction for the purpose of suurendering the Additional PAN alone. In this case whether I need to Provide all my PAN details such as Name, Address, DOB etc in PAN Correction Form ? Whether I can Fill the Correct PAN No(in the Top of the PAn Correction Form), and the PAN no which needs to be submitted(in the appropriate place) alone in the PAN Correction Form?

you dont need to fill pan correction form
just write a lettter to the ITO of your ward, quoting the additional pan requesting them to surrender the pan
they will verify the transsactions in your pan and then if there is no problem, will cancel the pan number alloted
