u have 2 write a latter to icai  with some information like  reg. no. , roll no. 

wrote to e -sahayta they wil give u some no. for contact

call them and comply the instructions given by them 

You write a letter with your hand writng(not printed one) specifing that your SRO Number, roll number, name, ditailes about your attempt etc, asking about mark list snd it along with a xerox copy of your hall ticket to the followng adderess. for detailes contact Board of Studies........

Main that letter should be as simple as possible is good not too formal.........
