1. Application to be submitted to Vikrikar Bhavan, Mazgaon, Mumbai-10 2. There are 2 types of registration (a) Registration Certificate: - for payment of tax deducted from salary of employees by employer-FORM I. (b) Enrolment Certificate: - for payment of tax by person / firm for himself- Form II. 3. The supporting documents are: a) Address proof, b) Details in respect of profession/trade/ calling/ employment and c) Photograph.
