no need to worry. you can apply for reprint of your PAN card  for which there is an separate form available in NSDL link given below:

https://tin.tin.nsdl.com/pan/correction.html

you have to file it with a copy of your PAN card or PAN allottment letter apart from your Id & address proofs.

When you have a PAN but have not received your PAN card/in case you have lost your PAN card, then you have to fill all the columns in the form for Request for New PAN Card or/and Changes or Correction in PAN data but do not have to tick any of the boxes on the left margin. In case of an individual, you have to affix a colour photograph of the size 2.5 cm X 3.5 cm.

No need for any proofs in case of lost cards (You should not tick anything in left margin).

NSDL never accepts that correction form which is not wid ID & address proof..

Fully agree with the procedure mentioned by you sir, but neither NSDL nor UTI accepts any PAN request without proofs as stated by Mr. Adarsh. So relevant proofs are required to be attached with the application even in this given case. 

first you should go for the FIR for the pan card lost and after then you may fill the correction form with all supporting documents with FIR copy and after then you get the pan card from the income tax department as like a new pan card like procedure.

Please clear that whether you misplaced PAN card and now you are not able to find the same or it has been stolen. If you go for the FIR,you may need to give valid reasons for such to Policeman.

What I suggest,you may just applied for Duplicate PAN via 49a Form(Correction Form).It is fully legal to ask for the Duplicate PAN card even if you have the origional PAN card.Moreover you dont need to submit reason for applying for Duplicate PAN card.You even not needed to do for FIR.So relax.Fill the form 49a(for Correction/Duplicate PAN issue),attach identity proof and residence proff along with Photograph.And submit to nearest PAN application center ( NSDL / UTI ).

Please note that you need to go through the same procedure as mentioned in above para even if you go for FIR.

In e-file, department demands for .xml file, and without generating this .xml file you cant e-file.

You do not have to bother !
All you have to do is to follow the step to get your Pan Card Download. That's it !
