Lost of goods in transit

sir I had supply goods to my customer but the same goods had been lost in transportation.The transactions took place in the month of November still today gst 3b had not been filed.what to do

1. Issue the Credit Note to Your client.
2. need to reverse the ITC on goods lost (in term of section 17(5))
