This is a very bad thing to have done. If u have already submitted the exam form, there is no way to go... your form will be rejected, So, better, go online and fill up the form in that and pay the exam fees with fine. Thats better than thinking about what to do more...

You could apply to the institute stating the reason for filling the form a second time... but as the reason for filling the form a second time is utter carelessness, the reply from the insitute would be negative.
