1. are the circles in the form be ticked or darkened?
2. what has to be filled up in state code coloumn? from where i can get these codes?
3. i havnt received my cpt marksheet, can i download my result from net and attach it?

1) You have to darken the circle.
2) Code means abbreviation of your State for eg. Madhya pradesh - MP, Uttar Pradesh - UP, Chattishgarh - CG. You can ask from your near Post office.
3) No need to attach CPT result, You have to mention your roll no. of CPT only.
As the form is not OMR, I have ticked the circle. The State code I have used is the 2 digit State code as per India (States and Union Teritory). I don't know, whether I have done corrrect or not!
