If you have form 16 - you should give a copy of it to ur AO and TDS AO also.

The onus of giving tax is on you employer.

its the fault of your employer who has either not paid TDS deducted or may have not filed proper return, ask them to make necessary corrections in TDS return so that it will appear in your 26AS
