Ignore, go ahead in completing the form. These minor things ICAI don't see them. The only thing ICAI check's is Medium, Centre, Groups applied and fees paid. So be careful while filling the above data.
As previously i said these are minor things. Please ignore them. The only major points are Centre, Group, Medium and fees paid. If you still have more doubt, kindly contact the email id given in the examination website for intermediate exam.
