Please tell me that in which form employee can declare its investment details to its employer ?

There is No Form Specified For this Under Income Tax Act.

Generally all Companies will have their Own Hr- softwares, in which Employee will give Details to the Company.

Legally speaking, Even if Employee Gives Details on 'White Paper' or Even 'Rough Paper', Employer is Bound to Take the Details of Such Savings.
