Income tax filling for freelance/proprietorship business

I am Software Engineer & work for a company. I also run a proprietorship business.

Do I need to maintain books of account?
Where can I mention the following income or expenses in ITR 4 (AY 2015-16)? I did not find any option there.
1. Internet bill
2. Apps purchases from third party for integrating to client's websites
3. Hosting and domain purchase expenses
4. Income from online advertisements

Freelance web development business.

Amount recieved from clients: 2lacs.
TDS deducted by client: 15K.
App purchased for client: 10K
Domain & Hosting: 30K

Income from online advertisements will be shown in other operating revenue.

Internet bill under other expenses. And hosting and domain purchases, apps purchases under purchases
