In DIN Application you can give:

Applicants First Name: Ali

Last name: Sefy

Fathers First Name: Mohammad

Middle Name: Husain

Last Name: Sefy

Bank Statement cannot be used as identity proof.

Prepare an affidavit for Fathers name –Single:

AFFIDAVIT

I Ali Sefy, residing at _______(Address as per address proof) do solemnly affirm and stated as under:

I am Ali Sefy and my father’s name Mohammad Husain , appearing on the enclosed ID proof, is single name. For applying DIN application of mine, I am mentioning my surname " _________" as my father’s Last name, as this a mandatory requirement for applying DIN. (Referred point no. 16 in FAQ at www.mca.gov.in ). Both names denote one and the same person.

I solemnly state that the contents of this affidavit are true to the best of my knowledge and belief and that it conceals nothing and that no part of it is false.

Batter to give bank statement than affidavit. I got din on bank statement.

First of all Bank Statement is not a Proof of Identity at all.

Secondly you may go with that PAN card because it is for your identity.

I think, there should not be any problem.

Because it requires id and residence proof of the Director and not his father.
