ICAI Reprint letter

I am not able to access my ICAI Reprint letter as it is showing a mismatching error. I want to take a print of the letter but the problem is that the form was filled up by my Institute faculty and hence I don't know the error what they had done while submitting the details regarding me. Can you please help me to have access to my ICAI Reprint letter.
