Huf pan card - date of birth?

The online HUF pan card form asked for date of birth - so I gave my (Karta) date of birth.

But the aknowledgement receipt showed Date of Incorporation/Agreement against my date of birth.

So, should the date be my date of birth or date of formation of HUF (next date of my marriage)?
