How to possibble assesse name changed on pan data base

Dear Sir / Madam,

my assesse PAN is ASMPP2633E and name is SREEDAR POJARI, I was filed his IT returns for the last asst year 2015-16 and paid tax. But to day on words IT wesite classified his name on PAN details CHAVA MURALI KRISHNA actually his not apply change / correction in PAN details.

How to possible this miracle, Please clearify me as early as possible.
